Abstract Background and Aim: Many underlie factors affects injecting drugs and numerous psychological and social factors are involved with the first injection experience. Thus the purpose of this study was to identify psychosocial predictors of initiation into injection drugs among substance abusers. Methods: The method of study was cross- sectional. Research Statistics constituted of all People suffering substance dependence disorder with pattern of injection use in the cities of Tehran and Hamedan. Study was conducted on 255 individuals with substance dependence disorder, selected randomly from harm reduction centers in Tehran and Hamedan cities. The tools used for data collection included: demographic information, patterns of drug use and initiation of injection scales and SCL90-R questionnaire. Data was analyzed by using multivariate regression analysis method. Results: Results showed that psychosocial predictors of initiation into injection drugs were; homelessness, unemployment, euphoria, aggression, curiosity, initiation age of drug use, peer pressures and faster effects of injection drugs. Conclusion: Injection drug use is a very complex phenomenon, associated with different factors which understanding those could be well applied to design more successful prevention approaches, or to postpone the initiation of injection.
